The Town of Bridgewater has been approved by the Department of State as a passport acceptance facility. While we do not process renewals (renewal guidelines), Passport Acceptance Agents at the Bridgewater Community Center (201 Green Street) accept applications for new passports or passports that have been expired for 5 years or more.

WHAT YOU NEED:
- COMPLETED APPLICATION (forms may be obtained from our office or filled out online and printed)
- PROOF OF US CITIZENSHIP (any of the following may be submitted)
- Previous U.S. Passport (undamaged; may be expired)
- Original certified birth certificate (from a city, county or state; NOT a hospital certificate; NOT a card)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization or Citizenship

- PROOF OF IDENTITY (any of the following may be submitted)
- Current, valid driver's license
- Military ID
- Government ID
- Certificate of Naturalization or Citizenship
- MINORS UNDER THE AGE OF 16 must appear in person and BOTH parents/legal guardians must present proof of identity (more important info on minors)
- PASSPORT PHOTO (we offer photo services)
- Passport photographs are free for town residents who present a B-Rec Membership Card.
- Photographs cost $15 for non-town residents.
